University Insurance Requirement |
The University of Minnesota requires:
All students admitted to the Academic Health Center are required to
be enrolled in:
- the University-sponsored AHC Student Health Benefit Plan; or
- a United States-based employer-sponsored health plan; or
- the University-sponsored Graduate Assistant Health Benefit Plan
All admitted international students are required to be enrolled in:
- the University-sponsored Student Health Benefit Plan; or
- the University-sponsored Graduate Assistant Health Benefit Plan
All other students admitted to a degree program who are taking 6 or
more credits (3 or more in the summer) are required to be enrolled
in:
- a United States-based health plan (could be a parent's plan); or
- the University-sponsored Student Health Benefit Plan; or
- the University-sponsored Graduate Assistant Health Benefit Plan
Students will be automatically enrolled in a University-sponsored
Plan unless they indicate they have some other type of health
insurance. As part of online class registration students are
presented with a screen asking for health insurance information.
Students who have health insurance must record their insurance
policy name and number when they register for classes.
Insurance information is audited by the Student Health Benefits
Office. If during the audit process information provided during
registration is found to be inaccurate, the student will be charged
a nonrefundable fee for the Student Health Benefit Plan. If you do
not have health insurance, you will leave this page blank, resulting
in automatic enrollment in a University-sponsored Student Health
Benefit Plan. A nonrefundable semester or summer session coverage
fee will then be applied to your student account. |

Coverage at Boynton Health Service |
Students admitted to a degree program who register for six or more
credits each semester are automatically charged the University's
Student Services Fee. A portion of this fee is allocated to Boynton
Health Service.
Students who pay this mandatory Student Services Fee and have health
insurance from any carrier (could be a parent's plan, an
employer-based plan, or on one of the University-sponsored Student
Health Benefit Plans) receive most services at Boynton Health
Service at no out-of-pocket cost or at reduced cost.
Here’s how it works.
All charges for services received at Boynton Health Service first
are billed directly to a student's health insurance carrier. For
most services, once Boynton receives an explanation of benefits or
payment from the student's insurance carrier, any charges not paid
for by the insurance carrier are then covered by the Student
Services Fee.
See the “Covered Services” section below for a list of services that
are fully covered or partially covered by the Student Services Fee.
The Student Services Fee only covers services provided at Boynton
Health Service. Check with your insurance carrier regarding coverage
for care provided at another clinic, hospital, or ER.
Students who are not eligible for coverage by the Student Services
Fee because they are not taking six or more credits may be eligible
to purchase Extended Coverage Benefits that provide the same
coverage as the Student Services Fee. See the Extended Coverage

Services Covered by the Student Services Fee |
|
Students who pay the Student Services Fee and have health insurance
from any carrier receive the following services at Boynton Health
Service at no out-of-pocket cost after the bill has been processed
by their insurance carrier. (Students enrolled in the
University-sponsored Graduate Assistant Health Benefit Plan may
incur a co-pay for these services.)
- Gopher Quick Clinic
- Primary Care Clinic appointment
- Urgent Care visit
- Annual eye exam (excludes exam for contact lenses)
- Women's Clinic appointment
- Pregnancy testing and counseling
- Smoking cessation coaching
- Lab tests (excluding fertility tests and blood typing)
- X-rays and minor surgical procedures
- Sexually transmitted infection testing and treatment
- Medical Information Nurse
- Specialty Clinic visit
- HIV testing and counseling
- STI testing and treatment
- Allergy shots (does not include serum)
- Nutrition counseling
- Select health promotion activities
- Birth control classes
- Stress management workshops (Tai Chi, Yoga, and Pilates)
- Financial /debt management counseling (first visit)
Students who pay the Student Services Fee and have health insurance
from any carrier receive the following services at Boynton Health
Service at reduced cost after the bill has been processed by their
insurance carrier. (Students enrolled in the University-sponsored
Graduate Assistant Health Benefit Plan may incur a co-pay for these
services.)
- Annual dental exam and x-rays
- Other dental services
- Mental Health Clinic appointment
- Physical Therapy Clinic appointment
- Massage Therapy Clinic appointment
- Immunizations and vaccines
- Contact lens fitting and follow-up care
- Contact lenses and glasses
- CPR and first aid class
- Physical required for various application forms
- Procedures not deemed medically necessary by Boynton medical staff
- Medications
- Dental treatment
- Services not medically necessary and not covered by your insurance
plan

Prescription Coverage |
|
Check with your insurance carrier about prescription coverage.
Students on any of the University-sponsored Student Health Benefit
Plans may speak with a Boynton Health Service pharmacist to
determine which prescriptions are covered by their particular plan.
If a prescription is written for a medication on the health plan's
formulary, the patient will be charged the co-payment designated by

Summer Coverage |
Spring semester students who need care in the summer will be covered
by the Student Services Fee at Boynton Health Service through mid
August the same way they were during spring semester.
Degree-seeking students registered for 3 or more credits during
summer term (1 or more for AHC students) will NOT be assessed the
Boynton portion of the University's mandatory summer term Student
Services Fee, provided they were charged for this fee spring
semester.
Degree-seeking students registered for 3 or more credits during
summer term who did not take classes spring semester will pay the
University's mandatory summer term Student Services Fee. Payment of
the summer term Student Services Fee means you will have summer
Student Services Fee benefits at Boynton Health Service.
Students registered for fall semester who did not take enough
credits during spring or summer term to qualify them for Student
Services Fee assessment may be seen at Boynton Health Service, but
their care will not be covered during the summer. Students insurance
plans' co-pays or deductibles will be the responsibility of the
student. |

Unresolved Claims |
Payment is due within 45 days from the date the claim is submitted. You will
receive monthly statements notifying you of the status of your account. You are
responsible to follow up with your insurance carrier for payment of your account
regardless of insurance coverage. Boynton Health Service does not follow up with
insurance carriers on unresolved claims. If your insurance carrier does not
respond, you will need to contact the insurance carrier to find out why they
have not responded and let Boynton Health Service know the status of the claim.
The most common reasons a claim is not resolved are that either accurate
insurance information was not provided during the Boynton Health Service
registration process, or if you have insurance through your parent you may need
to show proof of student status before they will process a claim. |
