Medication Therapy Management for UPlan Members |
Boynton Health Service is pleased to offer
Medication Therapy Management (MTM) services for
eligible UPlan members. MTM offers face-to-face
consultations with pharmacists specially trained
to help you improve the quality and
effectiveness of your medications.
UPlan members and their covered dependents who
take four or more UPlan-covered prescriptions
and covered over-the-counter medications for
chronic conditions are eligible for the MTM
program.
The UPlan pays the full cost of the
consultations with a Boynton Health Service MTM
pharmacist, and offers a six-month copay
reduction for covered medications. The copay
reduction amount is $8, which means no cost for
Generic Plus medications and a reduced copay of
$12 for Brand Name medications on the UPlan
formulary. (The $8 savings on just four covered
medications for six months comes to $192.)

bring a complete medication list from your
medical provider, or complete and bring the “My
Medication Record” form from
UPlan website to
your MTM appointment.
