Immunization Services
Immunization Requirements & Travel

Depending on the college that you are in, you may be subject to different immunization requirements. Identify student status (AHC or Non-AHC) from the list of colleges below. Then click on the links according to your status. You will find details on immunization requirements for the respective student status, as well as the immunization forms for downloading.
 

 

Academic Health Center (AHC) students are comprised of the following colleges:

- U of M Twin Cities:
Division of Dental Hygiene, Medical School and Graduate Medical Education, Physical Therapy Program, Program in Clinical Laboratory Science, Program of Mortuary Science, Program in Occupational Therapy, School of Dentistry, School of Nursing, School of Public Health, and School of Pharmacy. (Excludes
Veterinary Medicine).

- U of M Duluth:
School of Medicine and School of Pharmacy.

AHC students are required to have a health clearance as a condition of enrollment. This information must be completed to register for classes in the AHC. See details on immunization requirements.

 
If you are not in any of the colleges listed above, then you fall under the non-AHC student category.

Minnesota law requires all students born after December 31, 1956, who enroll in a Minnesota college or university to be immunized against diphtheria, tetanus, measles, mumps, and rubella. You are responsible for meeting the immunization requirements and holds that may apply to you. See details.
