According to OSHA regulations, CDC guidelines, and Academic Health Center (AHC) policy, all AHC students are required to have a health clearance as a condition of enrollment. This information must be completed to register for classes in the AHC.

Please send your immunization documentation either via postal mail or fax:

Fax to: 612-626-9768.

1. Go to www.myu.umn.edu and click on the link called "my Toolkit."
2. You will be asked to log into the portal using your University Internet id and password.
3. You will see your immunization information in your personal "To Do List" in the portal.

• If you have questions or need technical assistance to access the myU website (when checking the verification status of your required immunizations), contact the University Helpline at (612) 301-HELP.
   

• If you have questions or need assistance after submitting your forms and documentation and you do not find the information you need in myU (Toolkit link), you can contact Boynton Health Service at immunizations@bhs.umn.eduu or (612) 626-5571.

Note: The AHC Immunization form does not apply to Veterinary Medicine students. If you are a veterinary medicine student who has been directed here, please use the Non-AHC Student Immunization Form, or go to the immunization requirements webpage for non-AHC students.
