International Students & Scholars Health Benefit Plan |
|
University of Minnesota policy requires that all
international students, scholars and their dependents to have health
insurance while in the United States. Healthcare in
the United States is complicated and very expensive - one illness can
cost hundreds of thousands of dollars and financially devastate you and
your family. |
|
The University of Minnesota mandates that all
international students, scholars and their dependents be enrolled in the
University-sponsored Student Health Benefit Plan (SHBP) unless they are
eligible for a waiver. See waiver details.

International students and scholars are required to purchase the
University-sponsored Student Health Benefit Plan and must be covered
from the time they arrive at the University of Minnesota and have their
documents validated by International Student and Scholar Services (ISSS)
to the date they leave the University. |
Early enrollment and coverage
- If a person arrives at a time where they have become ill and
have not had a chance to check in with ISSS (i.e. arrive Friday,
became ill, was hospitalized and had not checked in with ISSS), they
should immediately call or check in with ISSS.
The Student Health Benefits Office will be contacted by ISSS to
facilitate the coverage.
- If an incoming student needs to access health services, they can
contact the Student Health Benefits Office (after they call the ISSS
office) to facilitate early enrollment as soon as August 1 with no
extra charge to the student

Waiving The Plan |
If you are covered by either of the
following, you will not be
required to purchase the insurance Student Health Benefit Plan.

A United States-based employer-sponsored health plan
or;
-
Graduate Assistant Insurance Plan (GA Plan) provided
by the University of Minnesota
|
Please bring the following to the Student Health Benefits Office at
Boynton Health Service:
- Proof of coverage such as your insurance identification card or;

Provide
a complete insurance information (name and address of company,
identification and policy numbers, name of
insurance subscriber, and phone number for us to
verify your coverage).
One of the above must be submitted to the
Student Health Benefits Office at 410 Church St.
S.E., Minneapolis, by the deadline*
in order to waive the charge for student
insurance.
*The deadline for
updating your insurance information
each semester
is the last day of the first week of class (this
date will be listed on the
One Stop website and announced in this website).

Health Insurance and Student Services Fee |
|
The University of Minnesota provides coverage for your health care
needs by offering one of the best and most comprehensive Student Health
Benefit Plans (SHBP) in the country. The combination of the SHBP and
Student Services Fee (SSF)
enables students to be seen at Boynton Health Service for:
Services received at a clinic other than Boynton are paid for at 80%
of eligible expenses.

Advantages of the plan |
- Coverage is guaranteed for incoming
international students, scholars and dependents from the time they
arrive at the University of Minnesota and have their documents validated
by
International Student and Scholar Services. Coverage continues as
long as the student or scholar meets criteria for eligibility.

The Blue Cross Blue Shield provider network: This is the broadest
provider network available, with 98% of the physicians and hospitals in
Minnesota participating in the network.
|
- The individual lifetime maximum benefit payment is $3,000,000. Your
annual, out of pocket expense is limited to $2,000 of covered services
processed for payment by Blue Cross and Blue Shield.
|
- The pharmacy benefit is provided to you and your family through the
Boynton Health Service Pharmacy with a maximum benefit of $2,000 per
plan year.
|
- Medical evacuation, repatriation coverage and travel assistance
services are provided by MEDEX Assistance. This is worldwide coverage
for medical evacuation in case of serious illness or injury and
repatriation of remains in the event of death.

- new! Accidental death benefit of
$10,000 US dollars for international students and scholars.

Visiting Scholars and Dependents |

Enrollment Procedures for Visiting International
Scholars |
- Visiting international scholars need to
complete the necessary enrollment forms and
return them to the Student Benefits Office at
Boynton Health Service,
within 31 days of arrival in the United
States.
- If visiting scholars are here for less than 31 days, this mandate
does not apply.
