Summer Coverage
1. Students with spring insurance coverage :
If you were enrolled in the Graduate Assistant Health Benefit Plan plan for spring semester, you will automatically remain enrolled for the summer unless you fill out a Cancellation of Coverage form. Download and print a copy of the form the Forms page.
2. Students with Graduate Assistantships:
You will receive a University contribution toward summer insurance coverage based on the semesters during the academic year in which you held a qualifying graduate assistantship.

If you are unsure about your eligibility or percentage of appointment time, please contact the department of the school that you are enrolled in.


Summer 2008 Insurance Premium Calculator
Find the total amount of insurance to be paid for the summer, including the University's contribution towards your summer premium.

Note:
This calculator is applicable only if you have were enrolled in the the Graduate Insurance plan for spring semester.

Note: Graduate students covered by Graduate Fellowships or department fellowships will be responsible for the entire summer premium, unless the department offers to pay for your insurance coverage.
Students who do not receive a full contribution for summer coverage will be billed for the balance on their Student Account Receivable.
