University of Minnesota Student Conversion Plan.
If you were enrolled under the University-sponsored Student Health Benefit Plan (SHBP) or Academic Health Center Student Health Benefit Plan (AHC SHBP) during the last semester, you will be eligible to enroll in the conversion plan.

Blue Cross and Blue Shield of Minnesota has teamed up with the University of Minnesota to offer students like you a guaranteed option for coverage called the University of Minnesota Student Conversion Plan. You qualify to choose from five plans offered by Blue Cross and Blue Shield of Minnesota if you enroll within 60 days of termination of SHBP or AHC SHBP coverage.

Please note, there is a section on the Enrollment/Change form that needs to be completed by the Student Health Benefits Office. Enrollment materials are also available in the Student Health Benefits Office located in room W323 of Boynton Health Service. (For spring and summer 2008 graduating students, enrollment forms must be received by Blue Cross and Blue Shield of Minnesota on or before October 17, 2008. Coverage will be effective retro-active to August 18, 2008).
