Your school is among 20 colleges and universities (see list below) across Minnesota partnering with Boynton Health Service at the University of Minnesota to administer the 2015 College Student Health Survey.
Complete the survey by noon on Friday, March 27, and you'll be entered into a grand prize drawing for a $1,000, $500 and $250 Amazon gift card!
To begin the survey, click on the survey link you received in your school or personal email account. Not sure if you received the email? Be sure to check your spam folder. And don't worry, we'll also be sending additional emails with your unique survey link.
The survey takes about 20-25 minutes to complete. And your responses are completely anonymous. If you need technical help opening the survey, please email firstname.lastname@example.org.

A random sample of students has been selected to participate in the survey. Emails with a link to the survey have been sent to selected students. Check your school or personal email account to see if you are one of the chosen few!
Reminders will continue to go out to these students throughout February and March.

Just for completing the survey, all participating students will be entered into a grand prize drawing for a $1,000, $500 and $250 Amazon gift card.
And one student from each school will have the opportunity to win a $100 gift card to Amazon.
Winners will be notified via email by Friday, April 3, 2015.
Those who complete the survey will be entered into the prize drawings. Although you are not required to complete all survey questions, you must visit all survey pages to be eligible to win.
