UPlan members, MTM services are available at Boynton—at no cost to you!

Boynton Health Service and UPlan Wellness are pleased to announce that one-on-one, in-person Health Coaching is now available to all UPlan members at no additional cost. Meet with a Health Coach 6 times in 6 months to receive a $65 Wellness Reward for completing a follow-up health improvement program, if you have not already completed one this calendar year.

Health Coaching is a unique opportunity for employees to meet with a clinician about the impact of lifestyle choices on health and how to make positive lifestyle changes.

Boynton Health Service offers three types of Health Coaching.

  1. Nutrition Health Coaching - available now!
    Weight control, high blood pressure, and high cholesterol are just a few topics that fall into the Nutrition Coach’s area of expertise. These sessions focus on assessment, establishing goals, and helping you attain your goals.

  2. Physical Activity Health Coaching – available now!
    There are numerous benefits of physical activity. The Physical Activity Health Coach can help you create a plan, and introduce you to exercises and activities that will help you meet your goals.

  3. Nicotine Dependence Health Coaching – available now!
    If you smoke or use smokeless tobacco (e.g.chew, snuf) the most important step you can take to improve your health is to quit. Our Nicotine Dependence Health Coach will help you figure out the best plan for you and will be there to offer support.

Just choose the type of Health Coaching you want and call to schedule an appointment. The first Health Coach visit is 30 minutes, and each of the five follow-up visits are 15 minutes.

To schedule an appointment, call (612) 625-3222. Please arrive on-time for your scheduled appointment. Those who arrive past the start time of their scheduled appointment will need to reschedule their appointment for another date and time.
