Patient Rights and Responsibilities

As a patient, you have choices, rights and responsibilities. You will not be required to waive your rights as a condition of obtaining health care at Boynton Health.

Should you have questions, concerns, problems, or complaints about the care or service that you are receiving, you are encouraged to speak to the health care provider or staff member involved in your care. If you prefer, you may complete a patient feedback form located throughout the clinic, speak with someone in the Quality Assurance Department 612-625-5688, or send an e-mail to bh-quest@umn.edu. Please include your name and phone number if you would like to be contacted.
